Insurance is unprofitable for Pend?

CCP has almost certainly considered it, but rejected it. Insurance is mainly intended as a new player ship reimbursement program. It is the CCP version of what some player corps now offer. I think if ship replacement programs from corps like eve uni become ubiquitous and cheap, then there might be an argument for removing insurance, or replacing it with some kind of new player only ship replacement program.

I agree with your sentiment that in the future, it would be neat to see EVE NPCs involved directly in less and less. This is the direction things seem to be going and I hope it continues.

If you can ensure that your ship blows up before the cost of reinsuring your ship becomes higher than the max payout, it is never a waste of money.
