My list:

Had my dreadnought pilot run out of game time while in warp to a C5 WH site with the fleet. Then logging it back in the next day, and having it warp into the site and trigger an escalation and require the fleet to rage log in and save in.


When rolling my static: Jump in a Moros, and collapse it out of my home WH with no scout, probes, or cloak. (It survived! Yay for friendly locals)

On a -10 pilot, jump into a high sec gate chasing gate campers. In a Deimos. (It died)

Jumped a Stratios into low-sec incursion gate rats (http://themittani.com/news/patch-day-carnage-first-stratios-destroyed)

Warped a 550mil isk Daredevil in to tackle a rail fit deimos. It was not rail fit. (Died)

Forgot to activate my C5 sites before warping in a solo naglfar. Landed 60km off the sleepers (Died)

Disconected right as I triggered my 2nd dread escalation in a C5. (Lived!!)

Sieged and bounced during a capital gank in my moros. Landed far away from our carriers, and next to a POS tower. Couldn't drop a depot to refit stabs. Swarmed by T3s. (Died)

just last night, I was trying to move a jump clone with my scan ship out where we intend to be mining gas clouds, came across 5 guys our alliance is at war with. This is only my third week in game, so I ran for a moon, cloaked up and then watched as about 25 guys came in through the system and out the gate I used to get in... I reported the movements and waited about 20 min with no more movement coming through, so I decided to finish my journey and jumped for the gate that they all came in from.... as I entered the next system I found myself surrounded by about 30 more reds camping that side of the gate. who instantly put up bubbles, killed my poor little Heron, and my pod.

Held a lowsec-lowsec gate with a known enemy support falcon on grid. Shot it for a bit. Eventually 2 of 3 of us jumped through... and found that the falcon blopsed in half a dozen T3's. Me in a Mega and the Navy Vexor burned back to gate and jumped through. They had a proteus waiting for us. I hit warp first, and accidentally hit stop JUST before getting into warp. Hit warp right away again, but got tackled by the proteus this time.

Shot the proteus for a bit, but it was too tanky. By this time the rest of their gang was jumping through. Falcon lit a cyno and brought in a thanatos to rep and put fighters on me. Deaggroed and boated through the stargate. Caught by a proteus on the other side. Had him in low low armor when 2 more prots jumped in to save him.

Oops.

Lost my shiny Kronos in 'Dread pirate Scarlett' mission Shocked

switched my security switchers to Red (thought only this way i could activate my guns even on suspects).... misclicked and gave some guy free killmail.... did this twice :D

and then was sitting in my shiny Daredevil on Dodixie undock.... when someone activated my KRs.....

oh LoL.... bought zydrine on 1.6k thinking that it will go up Lol

Seeing a ship floating during a POS bash and accidentally boarded it rather than show info. I was in a BS, it was a t1 frigate. Then had to ask fleet to wait while I got back into the right ship, which had flown off 90km or so. Oops

Any op where I log off cos its late then log in the next morning 3 regions from home, hopelessly lost in a BS. I've self destructed a couple of times rather than try get back, because maps are hard.
