I let a Rhea get away once because I double clicked my point and he'd just aligned to station as I jumped behind him.

That was a horrible loss. I may have sworn.

You know how when you're new you're tempted to let some successes go to your head, so you slowly add more, and more, and MOAR pimp to your favorite 'indestructible' ship over time?

My worst loss was my faith and trust in CCP. But with the recent direction they are taking in development, they start to gain it back.
