How does someone even get into your mission to kill you?

TBO whether you are safer in highsec v. low is questionable. It really depends on what you are flying. If you are flying a bling ship, e.g. a marauder with faction fittings, there is a significant possibility of being ganked in highsec. Conversely much, if not most, of low sec is empty and very safe to operate in. Ofc if you pick a lowsec system that is directly adjacent to highsec filled with pirates, you get what you asked for.

Well, the same can be said of high-sec. There are plenty of empty high-sec systems to be found.

I lived in one high-sec system in the past that never had a local count in the double digits unless I logged in the alts.

Unless your ship is really fat (capital-sized or something), the probes always have to get within 14 AU to get a scan on you. That means that yes, you will be able to see them on d-scan.

No. Often the acceleration gate remains unlocked. If it does lock, the mission keys can be found on the market quite cheaply. A locked gate can be an impediment to an unprepared ganker, but it's not insurmountable.
