Well, it's pretty clear to me. She's a doctor and gets confused with a doctor doctor while she is a not a doctor doctor. Maybe you should get another doctor, so people will see that you are a not a doctor doctor doctor and not a doctor doctor as doctor doctors usually only have a single doctor.

Once upon a time, a PHD insisted on being called Doctor Somename in the real world. Such as flying under the name Dr. Somename and insisting that the flight attendants address him as 'Doctor' instead of 'Mr.' He stopped doing that when there was a medical emergency on the flight and the flight attendants asked him to help.

Sorry, most folks will assume you're a medical doctor. Or, since this is a game, they may assume it's just a silly name or a little known literary reference, etc..

You could simply add that to your bio. You can mention what University you acquired your doctorate from and maybe steal a bit from the bio of this guy.

in english having a PHD grants the title of "Doctor",

you could perhaps say that you are a PHD, not an MD? (MD as a specification of medical doctor, rather than simply having a doctorate degree)

on a cultural note, ... how can a Medical Doctor be looked at in such a negative light? I mean, in China are medical doctors not supposed to be healers that make you better?

(I admit, perhaps thats an idealistic angle to take even in/particularly in the western world, but... well, lets go with that)

on another note, this cultural difference does make me think of how in Dr Who, different worlds sometimes have different meanings as to what the title/word "doctor" is, according to their experience with THE Doctor.
