Help me ID this Star Trek episode

Hoping someone has seen this and remembers the name.

I THINK it's from Voyager, but I'm not sure. In it, the crew (primarily Chakotay I think) is communicating with another Star Fleet officer that is extremely far away from the ship. They are trying to get to her, she's trapped on a planet or something. By the time they get to her, she's dead, because time had passed much more quickly on Voyager than the place where she was. She died of her wounds but her corpse appeared to be decades old when they reached it. It had something to do with relativity I think, but I don't remember.

Not sure if this is from Voyager. Just planning on catching up on Star Trek episodes through Netflix and I'd like to see the ones that I vaguely remember as a kid when the series was on TV.

I love that episode. It is so sad.
