Two factor authentication

A lot of us had trouble with TFA when Ascension was released. For me it was the fact that the authentication code sent to my e-mail address was not accepted. I had over fifty failed attempts before I was able to log on. Now it's happening again when I try to log on to Sisi. I've raised a petition about this and have been knocked back. No idea why, it's the same problem that the GMs were able to sort out the last time.

This raises two questions:

Why is any log on attempt failing, when I have already been authenticated against TQ?

Secondly why, when I have TFA switched off, am I still being sent faulty authentication tokens?

I'd like to be able to test some ideas I have but because of this I am unable to do so.

Does anyone have any idea what's going on here?
