Zero SP Bug, What causes it?

Dear CCP,

I know the Zero SP bug is either purely visual or something very temporary that's generally resolved by logging out. Then, after a long walk and some fresh air ;) re-logging back into the game.

But, seriously, - what causes it, technically speaking? Why does it only rarely happen?

I'm guessing it's related to session changes and your 'brain in a box' configuration for how characters exist on the server. Yet otherwise I've no clue.
