2 prime examples of "ye olde code" on EVE:

1st - the nebulae you see on missions? the one that kills your framerate? programmed by a guy on the beginning of times and nobody wants to touch it nowadays. It be old.

2nd - few years back, the boot.ini issue.

I don't always forget to specify a path but when I do I delete boot.ini.

Ask any oldtimer who runs a POS.

When a patch/expansion rolls by that even slightly hints at POS related changes, send in some new recruit to check on your POS, it might have gone skynet.
Quite some stories from past happenings where ships (even capital ships) have gone lost, due to POS turning their defenses on blues (or even your own corp/alliance friends!)

Hell, that's a good idea to do before any jump to a POS.
