Possible outbreak in Krygystan, just in case the Four horsemen want to make it an interesting week given we've got War on the topic, and now Pestilence.

The plague was never wiped out. It still exists around the world. Mostly moving through rodent colonies.

But clean living means it's hard to contract and since it's spread through bacteria. Most standard antibiotics clear it right out.

The very sun of heaven seemed distorted when viewed through the polarising miasma welling out from this sea-soaked perversion, and twisted menace and suspense lurked leeringly in those crazily elusive angles of carven rock where a second glance shewed concavity after the first shewed convexity.

the plague cycles through outbreaks fairly frequently

most people in the 1st world don't notice because of effective treatments
it's so mundane it's not even newsworthy anymore

Well it would have to have been exposed to an antibiotic to become immune to it and there are a lot of different types out there.

New strains are if anything more susceptible to antibiotics than any. The dangerous ones are just regular strains of bacteria that linger around hospitals and get constant exposure to antibiotics.
