At what percentage of damage should I replace my mining crystals? At this time one is at 18% and the other is 12% damage. Do I lose productivity as they get worse?

Productivity is unaffected by the damage.

They work perfectly, then they break and disappear.

Some people wait till they've close to breaking, then recycle them (for the nocxium that was used to make them). If you had good enough skills, you could get back all the materials used to make a T1 crystal. T2 crystals don't have anywhere near that refining efficiency (deliberately). But you could still get back around 20k on them with the right skills.

Replace them when they become consumed. Just keep some spare crystals in your cargo, since your ore is going into the ore hold anyhow.

When do I know they are close to breaking. Something like 50% or more? Thx for the info on productivity.

They break at 100%.

How much damage they take is random, but, statistically, you get a feel for it. Each cycle there's a chance of it taking damage. (The volatility, if I remember correctly). Over time, it evens out.
