Huh? 40% standard refinery? I thought 50% was the standard in high sec, and 35% was the standard outpost refinery.

I knew the Minmatar comes standard with a 35% refinery, and the other outposts can't refine until upgraded, but I thought that was just 1 upgrade option, a 35% refinery - what is this 10% upgrade, and 40$ standard business?

Yes but lets be honest it would have said 35% base before it was upgraded. The wording changes once you've added the first pedestal and upgrade. The relevance of 40% being that with near maxed skills and a 2% refining implant you can eliminate waste - no real reason to take the upgrades over tier one on a minmatar station as they get far more expensive over tier 1.

From the static DB, the number of high sec stations and their various refining efficiencies is:

25%..........19
30%..........263
32%..........168
35%..........55
40%..........12
50%..........4664

So, 19 of the 5181 high sec stations have 25% refining efficiency. The vast majority, 4664, have 50% refining.

The SQL used to derive the above list is:

If you need to visit one such station to verify that they exist, then head to Vale in Essence, "Vale VI - Moon 2 - Chemal Tech Research Center " is one. The other station in the system has 32% refining.
