I've had it occasionally happen on rats where you have to lock them twice because the first time just fails for no reason, but now it's happened in pvp and cost me a kill because you shouldn't ever have to plan for your locking time of being half what it actually is. I know you nullsec pilots must have a way because if it ever happened while on a carrier trying to rep someone I would definitely submit a reimbursement petition and then probably close my remaining accounts when they deny it for no reason.

PS: Why in god's name CCP do you need to submit your password for a bug report? Are you really so far bogged down that you need to artificially deflate the number of submissions? Also would be nice if you talked to your GMs every so often because I bet they could tell you exactly what the major issues in the game are at any specific time and potential fixes.

Do you mean the bug that lets you start locking a target before you exit warp, and then tells you targeting attempt failed?
