Something else wrong with him, being ex-military myself I can say I have been up for over 40 hours without sleep once or twice in my life.

Must admit I was much younger and much fitter then though.

It was probably a blood clot. 40 hours without sleep or food is pretty easy for a healthy human. But 40 hours stationary can get anyone via a clot, really just luck of the draw. It's why if you go on a long flights, there will be advisories suggesting you get up and walk around a few times during the flight, to protect against blood clots. It's also a concern for limbs that have been immobile in casts for a long time. Dude probably stood up, had a bad dizzy spell from exhaustion and passed out, came to, and then the clot hit his heart or brain. Very rare, but it happens. Sad story.
