On my first char I lost my one and only ship in one of the combat tutorials and didn't have the money to replace it. I had to make a new char.

I confess that I once accidentally caused the destruction of a corp-mate's ship in a WH. He was scanning down some sites for us, when a Loki and a Tengu showed up. He was in a Ferox, I was in a cloaked Manticore, watching D-scan for my mate.
My first reaction when the Loki showed up on D-scan: Run away as fast as possible!
My second reaction, after having made it out of the WH: Warn my corp-mate about the enemy.
My third reaction after receiving a "Where the hell did you go!?" in Fleet-chat: Tell him I would've warned him if I hadn't panicked. He's forgiven me since then, thankfully, since it was him who insisted on hiding on the warp-in point at the sun.

Yeah, I used to be an idiot. I'm smarter now. Now it's "warn, then warp".

When I first started playing, my friends from my old WoW guild invited me to come run a level 4 with them. After raping everything, they began dinking around and shooting at each other, which I thought was cool, and I wanted to join in on the fun. It was at that moment that I discovered that to CONCORD, there were two kinds of people: those that can fire on each other freely, and those who are violently repressed when they try to do so.

I confess I laughed one time when my neighbors kid fell out of his wheel chair and hit his head on a big rock.

This is EVE related because when he heard me laugh he called me a "Goon"...

One time, with my spy alt, I spotted a war target afk at the gate. I warped in, weapons hot / sensor boosted, and targeted... my own alt by mistake :( Alt died, main got concordokken, war target looted all.
