I was looking at Amarr Corporations and noticed Jamyl Sarum is still listed as the CEO of Emperor Family...

There was a Duvolle research agent I was working with who died in a chronicle a couple of years before they got around to replacing him - they didn't replace his Avatar though, same guy with a new name. I doubt CCP has anyone tasked with chasing the ripples when a NPC character dies, even one as important as Jamyl. Eventually someone will notice and it will get fixed.
