The MWD does indeed need to be off or cycling off when you leave cloak otherwise you will need to reach 75% of mwd velocity which you will be nowhere near.
I do:

Align

Cloak

MWD (immediately after, though in my experience you have at least 1 second, possibly even up to 3; but no need to risk it)

Uncloak when MWD cycle finishes (no need to cancel MWD, it auto cancels when you cloak. but it has to finish one cycle)

Warp (immediately after)

The only really important info is that it needs to be an Improved cloaking device and NOT a Prototype cloaking device. You will figure out the rest.
