I recently lost my ship to a POS, but in a very odd circumstance.

While exploring in Low Sec I warped to a moon, 100km off, and well, it warped me directly ontop of a POS. Needless to say, I was popped before I could warp off, because I was actually within a structure, so my speed was limited to about 25%.

Not too bummed about my loss, but, isn't there some sort of prevention for warping on top of POS? Well, whatever!

EDIT: Woops, guess the title wasn't very descriptive.

A few points:

The POS isn't necessarily anchored at 0 on the moon.

In Nullsec and WH's, people often put up warp bubbles to pull players warping to the moon to a spot of their desire.

A POS takes a pretty lengthy period of time to lock most ships (especially frigates). Generally speaking, you should easily get into warp before the POS attacks you. Don't hang around on grid with a POS!!!!

Dont just spam warp, manually fly into an open space and then try to warp next time.

Yeah, the yellow lock timer from the batteries were like a doomsday countdown in slow motion. Unfortunately, the problem was that even with all that time, traveling at 100m/s about, wasn't enough to get out of the structure and into 'space' to warp. Sad day.

THe easiest way not to warp into a POS is not to warp to moons. Works 100% of the time for me.

You can use your ship's built-in directional scanner to check for starbases before warping to moons; this is especially important to do e.g. when surveying moons for minerals in hostile space, as otherwise you may run into issues as mentioned above.
