So I logged into EVE not too long ago and found my character was celebrating pants-less friday. Or something. At first I thought I had gotten hacked, and as a cruel parting joke they had recustom'd my character. Of course I realized eventually that this wasn't possible. Then I tried to fix it in character creator, and this happened:

Hurray for...what?

Fast forward to now, and my chick seems to have lost her head...but not her facial piercings.

Oh god not the face!

It's been happening off and on since Inferno was released. It happens every 3 or 4 times I log in, my character is missing some...part...of her model.

The same thing that happened to you in the second screenshot happened to me more than once.

Just delete game cache, and problem should go away.
