if you dropped to meta 4 extenders would you still need the implant?

Doesn't need implant. But for maximizing EHP you should change one of the T2 invuls to named.

What are your skills and how much are you willing to spend? Thats a bigger question. With the right skills and a blank check you can get one hell of a tank on it.

ETA:

Its also important to be realistic as to what you are defending against. A single medium fit / skilled destroyer or a full fit and skilled gang of Catalysts?

but in my eye i think

Lows:

Mining laser upgrade II
Damage Control II

meds:

survey scanner II
Adaptive Invul Field II
Thermic Dissappation Amp II
Upgraded EM Ward Amp

High:

modulated strip miner II x 3

EDIT: (forgot rigs) 2x core defense field extender

Works pretty well against gankers while mining in high security space, but if they want to go after you, they will use something other than a destroyer to get you. So, Keep your eye on D-scan. Nothing is absolute. =)

2 destroyers can put this down in under 10 sec is they want your Hulk dead
1 Catalyst destroyer can put about 600 dps (damage kinetic/thermal in blasters)...
I learned this hard way...

34.3k EHP. If you want more yield with an MLU you'll lose tank. You might swap out a couple of those T1 strip miners for T2s with a 5% grid implant, but that's about it. Anything else sacrifices tank. If you get into a fleet where someone's got an orca or something packing a shield harmonizing link you'll eke out around 38.6k EHP. Omni at least. Swap the EM ward field for a kinetic deflector if you're worried about catalysts.

Basically what Shereza suggested but with a MLU on it. Tank should be enough to scare off most of the gankers, but if someone wants you dead you will die anyway.
And to be honest... if you want pure tank, you should pick a Skiff as mentioned above. A bit harder to manage the single miner but you can be pretty sure that a ganker is rather picking on the Mack/Hulk in the next belt instead of trying to pop a Skiff.
