You go from trial to subscribed account by just paying for the game.

You can do this in the following ways:

1.) Buy a PLEX from the in game market (around 500mil ISK) and then use it to extend your game time.
2.) You can subscribe by selecting one of the payment methods on the account management page
3.) You can buy Game Time Codes (GTC) from for instance ShatteredCrystal or Battleclinic

4.) You can buy a PLEX from CCP directly and use that PLEX to extend your game time. PLEX bought from CCP will show up in your redeem items of the escape menu.

Any of those ways (ergo, you pay money to play the game) means your trial converts into a subscribed account.

How much will this cost you, that depends what type you use. (keep in mind that I matched the same size of payments to make comparing them easier).

Shattered Crystal sells GTC (2 PLEX = 60 days of gaming) for $35,-
CCP sells 2 PLEX (60 days of game time) for 35,- Euro (which is around $46,- with current exchange rate)
If you subsribe form the account page directly you get discounted if you take longer subscribtions.

There is the possibility that you are charged a fixed subscription fee for the first month.
